Drinking water systems that are regulated by the Ministry of the Environment need to be registered with the ministry and information about the system kept up to date.
If you need to register your system, complete and submit these forms:
Email the drinking water system profile information form to the ministry at reg170_formsubmission.moe@ontario.ca. You will receive a confirmation letter or email with your drinking water system (DWS) number when the form has been processed. Once you receive a drinking water system number, complete the lab services notification form and fax it to the ministry at 416-235-6312.
Year-round residential systems, as well as systems that serve designated facilities, must also complete these forms:

If you need to update information about your system, complete the drinking water system profile information form and submit it to reg170_formsubmission.moe@ontario.ca.
By registering your drinking water system and maintaining information about it, you ensure better communication between you and others involved with safekeeping our drinking water – including laboratories, public health units and the ministry.
