Prior to sampling for lead, you must register your school, private school or day nursery with the Ministry of the Environment by completing and submitting the Registration and Laboratory Services Notification form.
In order to complete the form, you must first contact a licensed laboratory which will test your samples. Information about this lab is needed for Section 4 of the form.

Once you have completed the form, email it to Ministry of the Environment at reg170_formsubmission.moe@ontario.ca. You will receive a confirmation letter or email with your drinking water information system (DWIS) number when the form has been processed. You will need to reference this DWIS number in future when dealing with the Ministry of the Environment.
