Director Notification Form - Alterations to a Drinking Water System
Description: This form is for drinking water system owners and is needed when alterations are being considered to a drinking water system and if Director notification is required by a condition of the drinking water works permit within thirty (30) days of the placing into service or completion of any addition, modification, replacement or extension of the drinking water system.
